Event Scheduling Form

This form is used to request a date on Shady Grove’s Calendar. In the event that the date requested cannot be met, you will be contacted. A deposit of $25.00 for members, or $50.00 for non members will be required for all non-church activities, and will be refunded in the event that there is no damage to church property and/or no extra cleaning is required by personnel. If additional cleaning is required by personnel, there will be a $100.00 cleaning fee. A Church Member is required to be in charge. Please keep in mind that parties reserving the kitchen are are asked to supply their own plates and napkins, cups, and silverware etc. We ask that upon returning the key, or finishing up with your event, that you submit the attached check list, completed, to the church office to receive your deposit refund.

Shady Grove Baptist Church provides meeting space for members and non-members to hold non-church functions as a service to the community. Keep in mind that providing this service comes at a cost to the church, and we kindly ask you to help us keep those costs to a minimum. You and your group agrees to comply with all of the rules and regulations regarding the use of the facilities of Shady Grove as set forth in this policy. In the event of damage to the church property during the group’s use of the facilities, the group scheduling the facility shall pay for such damages. Any building deposit paid by the group will be applied to such damages. In the event that the group does not satisfy the expectations of the Cleanup Checklist (attached), I understand that the church can apply my deposit to the cost of re-cleaning the building.
