Event Scheduling Form
This form is used to request a date on Shady Grove’s Calendar. In the event that the date requested cannot be met, you will be contacted. A deposit of $25.00 for members, or $50.00 for non members will be required for all non-church activities, and will be refunded in the event that there is no damage to church property and/or no extra cleaning is required by personnel. If additional cleaning is required by personnel, there will be a $100.00 cleaning fee. A Church Member is required to be in charge. Please keep in mind that parties reserving the kitchen are are asked to supply their own plates and napkins, cups, and silverware etc. We ask that upon returning the key, or finishing up with your event, that you submit the attached check list, completed, to the church office to receive your deposit refund.
